I think the folded hands are fake, that is why they always stay folded. The rest was creating the illusion of a head falling off by either bending over or kneeling.
There is a wire frame for the shoulders that the magician’s real hands prop up after the collapse to the floor.
I’m just kidding, that was a really cool illusion though. I was surprised the first time I watched it.
